What is Extracorporeal Focused Shockwave Therapy?

At Activate Chiropractic, we are always looking for advanced, research-backed therapies that support the body’s natural healing processes. One of those therapies is Extracorporeal Focused Shockwave Therapy (ESWT).

Focused shockwave therapy uses high-energy acoustic (sound) waves to stimulate healing in injured or degenerative tissue.

Unlike surface-level treatments, focused shockwaves are directed through a specialized lens, allowing the energy to reach deep structures in the body, up to nearly 4.7 inches beneath the surface. This makes it possible to treat deeper tissues such as tendons, ligaments, muscles, joints, and fascia.

Most importantly, these sound waves do not simply “massage” the tissue. They activate powerful repair mechanisms at the cellular level [1].

How Shockwave Therapy Triggers Healing

1. Mechanical Stimulation: Signaling the Body to Repair

When focused shockwaves enter tissue, they create rapid cycles of compression and tension. This gentle mechanical stress:

  • Improves tissue mobility
  • Stimulates circulation
  • Encourages cellular responsiveness

Your cells interpret this mechanical stimulation as a signal that repair is needed.

This process is called mechanotransduction, which simply means converting mechanical force into biological healing activity.

2. Turning Sound Waves Into Healing Signals

Your cells are designed to sense movement and pressure. When shockwaves reach them, several healing pathways activate.

Cellular Communication

Special structures on the surface of cells respond to the mechanical stimulation and trigger internal repair pathways. This promotes:

  • Cell survival
  • Tissue regeneration
  • Growth factor production
Release of Healing Messengers

Shockwave therapy also stimulates the release of tiny biological messengers called microvesicles and exosomes.

These microscopic messengers:

  • Promote new blood vessel formation
  • Accelerate tissue repair
  • Improve healing in areas with poor circulation

Research shows that these exosomes carry regenerative genetic material that helps coordinate and amplify the healing process [2].

The Biological Effects of Focused Shockwave Therapy

Once these healing signals are activated, several important processes occur.

Increased Blood Flow (Angiogenesis)

Shockwave therapy stimulates the formation of new capillaries from existing blood vessels [2]. This process is called angiogenesis.

Improved circulation means:

  • Better oxygen delivery
  • Increased nutrient supply
  • More efficient waste removal

Healthy blood flow is essential for long-term tissue repair.

Recruitment of Healing Cells

Shockwave therapy also releases signaling proteins that attract healing cells from within your body to the injured area [3].

These cells help:

  • Rebuild damaged tissue
  • Support blood vessel formation
  • Restore structural integrity

This allows healing to occur from the inside out.

Regulation of Inflammation

Inflammation is not always a bad thing. In fact, it is necessary for healing when properly regulated.

Focused shockwave therapy helps coordinate the immune response by:

  • Clearing damaged tissue
  • Activating regenerative signaling pathways
  • Encouraging healthy tissue remodeling

Rather than simply suppressing inflammation, shockwave therapy helps the body complete the healing process properly [4].

Cellular Reprogramming, Growth Factors & Long-Term Repair

One of the most exciting aspects of focused shockwave therapy is its ability to shift tissue from a chronic, degenerative state toward true regeneration.

In long-standing injuries, tissue often becomes fibrotic or scarred. Focused shockwaves stimulate cells to express repair-related genes, increasing their regenerative potential rather than continuing a cycle of breakdown and compensation.

At the same time, shockwave therapy stimulates the release of powerful healing compounds, including:

  • Growth factors

  • Regenerative signaling proteins

  • Cellular energy molecules (ATP)

Together, these processes promote:

  • Collagen remodeling

  • Stronger, more organized tissue repair

  • Improved tissue quality

  • Faster and more complete recovery

The result is not just temporary symptom relief, but structural healing that supports long-term function and resilience.

Is Focused Shockwave Therapy Right For You?

Focused Shockwave Therapy is commonly used for chronic tendon injuries, plantar fasciitis, shoulder and hip pain, tennis elbow, joint degeneration, and stubborn soft tissue injuries that have not responded to traditional care.

It is safe and well tolerated for most patients. Temporary soreness, redness, or mild bruising can occur, but these effects typically resolve quickly. However, treatment should not be applied directly over lung tissue, malignant tumors, a fetus, pacemakers or defibrillators, or areas with significant bleeding disorders.

If you are dealing with chronic pain or an injury that simply has not healed the way it should, focused shockwave therapy may help stimulate the repair process your body has been missing.
